Hospitals and ACOs are building partnerships with organizations such as hospices that provide palliative care services on an out-patient or in-home basis. The partnerships are designed to reduce hospital readmissions, mortality rates and treatment costs among this patient population.

Leaders from Baptist Health and Hospice of the Bluegrass share tested strategies for building effective hospice-hospital partnerships to improve clinical and financial outcomes.

Anne Monroe, MHA, is Physician Business Practice Manager for Hospice of the Bluegrass and Palliative Care Center of the Bluegrass (HOB/PCCB).  She has worked in physician practice management for 20 years.  She completed her Master in Health Administration at the University of Kentucky, and is a member of Medical Group Management Association (MGMA).  Ms. Monroe is responsible for program planning, budgeting, system and data analysis, regulatory compliance, clinic operations and other physician practice management responsibilities for the HOB/PCCB practice of 12 physicians and 12 nurse practitioners.
