Effective care coordination begins in the primary care practice. The leaders of Geisinger Health System understood that to drive costs out of the care continuum, they had to be able to navigate patients effectively among the appropriate healthcare providers. They began by leveraging the power of their ProvenHealth Navigator™ model, a system that combines evidence-based best practices, electronic health records, and quality and accountability metrics to standardize care processes. Primary care physicians and nurses have been supplemented with RN primary care managers for patients with chronic conditions. These care managers build relationships with patients and coordinate support. Combined with the other population health and HIT tools, the work in care management and primary care has achieved dramatic results. Overall admissions for Geisinger Health Plan patients have dropped 18%, while inpatient readmissions have been reduced by 36%.

Hans P. Cassagnol, MD

Associate Chief Quality Officer of Geisinger Health System, Director of Obstetrics and Gynecology at Geisinger Medical Center Northeast

Dr. Cassagnol joined Geisinger in 2002 and also chairs the Performance Improvement Committee and co-chairs the Ethics Committee. He is a member of the teaching staff for the Geisinger Obstetrics and Gynecology Residency program and has served on the ProvenCare® Perinatal Initiative. Dr. Cassagnol earned a Bachelor of Biology degree from St. John’s University, a Master of Medical Management from the University of Southern California, an MD from the University of Connecticut School of Medicine, and completed his residency at Flushing Hospital Medical Center.

Elizabeth Price

Senior Director, Care Gaps and Community Practice Best Practices, Geisinger Health System

Elizabeth “Liz” Price is a Senior Director of Care Gaps and Community Practice Best Practices within the Geisinger Health System. She is accountable to direct the development, implementation and dissemination of primary care innovation initiatives, specifically as they relate to streamlining the use of the electronic health record, point of care workflows and pre-visit workflows across 45 primary care practices. At a system level, she is responsible for leading and managing the identification and closure of clinical care gaps across primary and specialty care. She is focused on achieving best practices, and following evidence-based guidelines with emphasis on improving quality, patient safety and clinic operations.

Liz’s 23 years of healthcare experience include clinic operations; electronic health record development, implementation, support, and performance improvement; and project management at both the strategic and individual project level.

Liz is a graduate of Misericordia University with a BS degree in Health Care Management.

Joanne Sciandra, RN, BSN, CCM

Vice president of Population Health, Geisinger Health Plan

Joanne Sciandra is responsible for program development, implementation, and training of case managers for Geisinger’s program, and also for external groups through the consulting arm of Geisinger Health System. A Certified Case Manager, Sciandra is involved in community education programs as well as educational programs for case management staff. Her experience includes nursing practice in the ICU as well as serving as geriatric clinic coordinator. More recently, she has been involved in case and care management as a case manager, preceptor, and team leader in care coordination. She received her BS in nursing from Wilkes University.

Jo Ann Sevison, MBA, MS, RN

Vice President, Population Health Management, xG Health Solutions

Joey Sevison is responsible for all resources and services for the design, implementation, and delivery of integrated population management at xG Health Solutions, a Geisinger affiliate. Sevison joined xG Health from Geisinger Health Plan, where she served as director of clinical solutions. Earlier in her career, she served as vice president of physician practice management for a large multi-specialty physician practice group. Her healthcare experience has included leadership roles in such areas as physician integration, human resources, recruitment, and nurse management. She is a graduate of the Geisinger School of Nursing and holds a BS in business administration from Bloomsburg University, an MS in human organizational development/human resource management from Villanova University, and an MBA from Eastern University.

Joan E. Topper

Vice President, IT Optimization, Division of Clinical Innovation, Geisinger Health System

Joan E. Topper directs the development and expansion of Geisinger's innovation initiatives for streamlining use of the EHR and IT applications to improve quality, patient safety, and clinical operations. As a primary facilitator in transforming the delivery of healthcare using health IT as an enabler, she is responsible for the patient and physician portals and the system's clinical data warehouse operation. Topper’s 25 years of healthcare experience include clinical performance improvement, healthcare finance, and clinic and hospital operations. She earned a BS in health administration from The Pennsylvania State University.
