Speakers

Christina Crumbley Christina Crumbley, RN, MSN, is the Chief Executive Officer of Tidal Healthcare Consulting, LLC, based in Tampa, Fla. She has over 30 years of experience in healthcare, with the majority of her experience in home health. As CEO of Tidal Healthcare Consulting, LLC, she consults home health providers on key operational and clinical processes, including OASIS review, ICD-10 coding and more. Crumbley is passionate about educating home health leaders on improving outcomes, including how to leverage documentation and mentorship programs to drive and support high-quality care. Over the past five years, she has traveled as a contract and part-time surveyor for the Accreditation Commission of Health Care (ACHC). She also currently serves on the board of the Home Care Association of Florida.
Melinda A. Gaboury Melinda A. Gaboury, COS-C, CEO of Healthcare Provider Solutions, has been in home care for more than 30 years. She routinely conducts home care and hospice reimbursement workshops and speaks at state association meetings throughout the country. Melinda has profound experience in Medicare PDGM, billing, collections, case-mix calculations, chart reviews and due diligence. UPIC, RA, ADR & TPE appeals with all Medicare MACs have become the forefront of Melinda's current impact on the industry. She is currently Chair of the NAHC/HHFMA Advisory Board and Work Group, Treasurer on the Home Care Association of Florida Board of Directors, and a member of the Tennessee Association for Home Care Board of Directors. Melinda is also the author of DecisionHealth's Home Health OASIS Guide to OASIS-E and Home Health Billing Answers.
Kelly Kavanaugh Kelly Kavanaugh, RN, HCS-D, HCS-H, HCS-O, BCHH-C, independent home health and hospice consultant is a registered nurse with over 30 years clinical and leadership experience in the home health and hospice industry. By serving in capacities such as a field nurse, quality improvement manager, educator, director of clinical services, administrator and agency owner, it has developed into a passion for the industry. She has been certified in OASIS and home health coding since 2005 and Hospice coding certified since its inception in 2014.  Kavanaugh is a current member of the AHCC Advisory Board for coding compliance, a frequent collaborator for DecisionHealth’s Diagnosis Coding Pro and the Home Health Line publications and is a current member of DecisionHealth’s Hospice Coding Exam Committee.
Annette Lee Annette Lee, RN, MS, COS-C, HCS-D, founder of Provider Insights, Inc., is a registered nurse with a master’s degree in healthcare administration, practicing primarily in the home health and hospice space for over 30 years. Her previous experience includes more than 10 years in Medical Review with the CMS Medicare Administrative Contractor (MAC), reviewing home health and hospice payment and documentation. Annette then founded Provider Insights, Inc. to provide consultation and education regarding Medicare reimbursement issues and effective documentation strategies, assisting providers with ADRs and surveys. Today, she marries her real-world experience and her inside knowledge of Medicare to ensure providers can meet CMS requirements and ensure compliant, efficient operations.
Lisa Selman-Holman Lisa Selman-Holman, JD, BSN, RN, HCS-D, COS-C, is the Vice President of Clinical Services at McBee. She brings over 35 years of experience in home health and hospice care. She's a nurse, attorney and regulatory compliance specialist with career roles that evolved from field nurse to executive, where she has adeptly managed home health accreditation surveys, acquisitions, and regulatory challenges. In her current role, Lisa oversees the clinical aspects of consulting projects, offering expert guidance on clinical operations, quality improvement, patient care standards, and regulatory compliance. She also plays a pivotal role in business development, identifying potential clients and crafting consulting proposals tailored to their clinical needs.
Apryl Swafford Apryl Swafford, COS-C & HCS-O HCS-D, HCS-H, who currently serves as QA manager for SimiTree, has more than 25+ years of experience in home health. She began her nursing career in 1994 with a BSN from Jacksonville State University in Alabama and has worked in roles as a case manager, clinical manager, and leader of a cardiopulmonary specialty program. COS-C & HCS-O HCS-D, HCS-H). She has led multiple coding and OASIS classes for agencies, as well as speaking on topics of interest to home care and hospices agencies, such as OASIS E, home health regulations, PDGM, HH VBP, outcomes management and others, at many state associations.
