Correctly capture information around the new and revised social determinants of health (SDoH), including health literacy, transportation, and race, and your efforts will pay off in terms of improved care plans and better quality outcomes. This webinar will share tips for how to document this information through new and revised OASIS-E items and ICD-10 codes.

In this 60-minute live webinar, Sherri Parson, HSC-D, chief compliance officer and director of operations with Infusion Health in Ypsilanti, Michigan, will discuss assessment strategies to capture the complete picture of a patient as well as how to create consistency between SDoH ICD-10 codes and OASIS answers.

Discover care planning tips around SDoH issues found in the assessment to make sure your patients have the resources they need to stay safe at home and to reflect positive quality outcomes as CMS rolls out national home health value-based purchasing.

At the conclusion of this program, participants will be able to:

  • Achieve compliance with proper use of SDoH codes for health literacy, race, and transportation
  • Avoid future audit scrutiny through transparency related to SDoH and documentation to back it up
  • Maintain consistency between the SDoH codes and OASIS answers
  • Determine areas in which your agency needs to improve to support patients with SDoH issues
  • Identify resources and tools to promote a strong, health-literate organization
