As of October 1, 2018, the Skilled Nursing Facility (SNF) Value-Based Purchasing Program (VBP) rewards SNFs with incentive payments for the quality of care they deliver to their Medicare beneficiaries- with particular incentives for reducing hospital readmissions. To avoid negative payment adjustments, facilities must have practical strategies in place to meet the program’s requirements.

During this 60-minute webinar, expert speaker Reginald M. Hislop III, PhD, Managing Partner and CEO of H2 Healthcare, LLC, will review requirements of the SNF VBP and help providers implement strategies to meet the SNF 30-day all-cause readmission measure. He will also identify solutions for key elements that cause readmissions, including balancing shorter lengths of stay with quality care under the Patient-Driven Payment Model (PDPM). Audience members will have the opportunity to submit questions to be answered live by Hislop after the show

At the conclusion of this program, participants will be able to:

  • Identify the steps that lead to readmissions and what can be done to lessen or eliminate the risk
  • Increased awareness of the tools available to reduce the risk of readmissions
  • Use best practices to improve care coordination
  • Know which key elements produce readmissions and how to limit or remove them, including medication reconciliation, care conference structure/strategy, care pathways, disease management programs, and communication tools
  • Utilize risk management strategies to balance appropriate lengths of stay with necessary reimbursement

  • The SNF environment and how VBP, star ratings, bundled payments, and reimbursement will be adversely impacted by rehospitalizations
  • How the SNF VBP program works from a mechanics and metrics standpoint
  • The continued expansion of rehospitalizations and care coordination scrutiny
  • The reputational and marketing impacts tied to rehospitalizations, and how they cause reductions in market share
  • The operational costs associated with rehospitalizations, including balancing shorter lengths of stay with quality care under PDPM
  • Live Q&A

  • CVA Pathway and Plan of Care
  • Estimated Length of Stay (Primary Orthopedic Dx) Tool
