Presented on: October 15, 2015
Presented by: Joan L. Usher, BS, RHIA, COS-C, ACE, AHIMA-Approved ICD-10-CM Trainer
Now that ICD-10 is in effect, it’s imperative that clinicians document appropriately both on the OASIS and in their clinical narratives to allow for accurate and complete coding. Without proper documentation, incorrect or insufficient codes will be assigned. In this 90-minute program, coding expert Joan L. Usher, BS, RHIA, COS-C, ACE, AHIMA-Approved ICD-10-CM Trainer, discusses common issues in clinician documentation and how these mistakes can affect overall reimbursement. By reviewing specific case examples, Usher describes ways the documentation and codes can be improved.
At the conclusion of this program, participants will be able to:
- Identify common documentation issues that lead to inaccurate coding
- Identify three strategies to effectively educate clinicians on supportive documentation
- Discuss four Test Your Knowledge cases to determine the level of specificity and laterality required to assign the correct code
1. Know your current state of documentation

ii. The clinical narrative?
b. Common problems:
2. Review documentation for:Consistency
