Primary care redesign presents an opportunity to address a key factor in improving outcomes and reducing costs: mental health. Intermountain Healthcare saw an opportunity to address the problem by integrating mental health services into the primary care model.

The organization developed a team-based approach that ensures the effective assignment of mental health professionals, along with a care process model built upon a systematic, evidence-based treatment algorithm that is collaborative and measurable. The mental health integration tools are embedded into the systemwide electronic health record, enabling team members to use the same screening tools, communicate with each other via notes in the patient record, and track results. The new approach has not only reduced costs, but also improved the patient experience.
