Arnie Cisneros, PT
The Affordable Care Act focuses on elements of the current care continuum and rewires delivery in terms of episodic care programs. In this 90-minute webcast, expert Arnie Cisneros, PT, will discuss how your home health agency can reduce elements of the silo behavior that currently limit care efficiency and outcomes. The key to care transitions management is an elevated level of care coordination that will be addressed through communication and documentation. What will be required from home health care delivery that doesn’t exist today? This presentation outlines the care concerns in today’s programming and delivery, and proposes alternative protocols that you can install to prepare for the care demands of tomorrow.
