Course Outline/Agenda

The Documentation Boot Camp for Hospital Residents consists of one eight-hour day (less a one-hour lunch and two 15-minute breaks) that can be customized for your individual needs.

  • Risk Adjustment

Module 2: Converting documentation to code assignment (1.5 hours)

Learning Objectives

Upon completion of this course the attendee will be able to:

  • Explain the basics of Medicare Part A & B reimbursement
  • Identify how reported diagnosis codes influence reimbursement, E/M (professional fee billing), quality measures, risk adjustments, and publically reported data
  • Explain the process of converting documentation to code assignment
  • Identify desired documentation practices related to diagnosis specificity
  • Communicate medical decision making and patient complexity in documentation of:
    • History & physicals
    • Progress notes
    • Operative notes
    • Discharge summaries
  • Identify how CDI efforts can assist in capturing effective, accurate and complete documentation.
