Writing compliant queries in the outpatient arena is a skill that requires much practice to master. Join Laurie L. Prescott as she discusses the nuances of crafting a query as well as best practices for querying providers in the outpatient setting. She will also review the 2022 ACDIS/AHIMA Guidelines for Achieving a Compliant Query Practice brief and provide tips useful to both new and experienced CDI specialists looking to sharpen their querying skills.

Regulatory Headwinds: Medicare Advantage and Value-Based Care Mantra For Success
Khush Singh, MD, MHA, CPC, CRC, CPCO, CPMA, CDIP
CDI professionals must be aware of the regulatory and documentation requirements at play in order to perform their roles effectively. This session will provide an overview of the latest updates to the Medicare risk adjustment program, including findings from the Office of Inspector General and Health and Human Services audits and the CMS recommendations and action plans to address coding compliance issues. Attendees will then learn how these audits and recommendations have implications for the False Claims Act and will impact the future of the Medicare risk adjustment program, hierarchical condition category coding, and value-based care.

Knowing where to start with your outpatient CDI program can be baffling, which is why it's crucial to hear from organizations who have driven the course before and came out as winners. In this session, attendees will learn how Northwestern Medicine started with a foundational ambulatory CDI program and scaled it to support the system's 100,000 Medicare Shared Savings Program lives and more than 440 primary care and internal medicine physicians, and gained the tools, people, and systems necessary for successful implementation.

Using Data Analytics in the Development of a Hospital–Based Outpatient CDI Program

Data analytics provides a useful tool in evaluating where the biggest "bang for the buck" is in an organization's outpatient service areas. Part of the process in developing and growing an outpatient CDI program is recognizing and prioritizing those departments with the biggest opportunity to improve revenue and medical necessity capture and decrease denial risk. This presentation will explain how to use data to eliminate the guesswork involved in identifying outpatient department focus areas when starting a program in a hospital outpatient system.

Practical Understanding of Natural Language Processing in Risk Adjustment Technology

Artificial intelligence (AI), machine learning (ML), and natural language processing (NLP), sometimes referred to as natural language understanding (NLU), have become inherent to applications promoting more effective and efficient HCC capture. While there is little debate over the use of technology tools to drive outpatient clinical documentation program performance, understanding more precisely how NLP applications work to identify potentially capturable HCC codes is crucial to the success of any technology application. In this session, attendees will learn the background of AI, ML, and NLP and how this emerging technology can be expected to perform in a risk adjustment–focused outpatient CDI program.

Whether an outpatient CDI program is established or brand-new, there are several key drivers that require the program to adapt and change continuously. Often, outpatient CDI programs are built as they go, dealing with challenges and adapting as they grow. In this session, attendees will learn how to navigate the four stages—discovery, introduction, rollout, and growth—in an outpatient CDI program's life cycle and take away strategies for continual growth and change, regardless of their program's maturity.

Understanding the role of social determinants of health (SDOH) codes in the physician practice setting is vital for pay-for-performance methodologies and hierarchical condition category coding. In this session, participants will gain an in-depth understanding of SDOH codes and learn how to convey their importance to others.

As risk adjustment empanelment volumes increase each year, the role of outpatient CDI becomes more important for capturing the full burden of patient acuity. Ensuring an outpatient CDI program is functioning to the highest level of performance is imperative to value-based care. In this session, learn how Baylor Scott & White evaluated access, utilization, and cost to identify opportunities for strategic program expansion to specialty care, while engaging key stakeholders (including health plans, when possible) to achieve system performance.

Developing and Growing a Pediatric Outpatient CDI Program

Pediatric hospitals are rapidly signing value-based care agreements with payers, but pediatric care is not included as part of the CMS hierarchical condition category (HCC) system. Instead, pediatric care is better represented by the Health and Human Services HCC program, and many payers do not even require HCC billing, instead requesting all the acute and chronic comorbid diagnoses identified for risk score creation. In this session, attendees will learn how the outpatient CDI program at Nemours Children's Health System has worked to meet the needs of the organization's value-based care contracts, without networking opportunities in the outpatient pediatric CDI world.

Speakers

Bernadine Cope, RN, CPC, has over 27 years of experience in the nursing profession and has spent the past 17 years in the CDI profession. She is a AAPC-Certified Professional Coder (CPC) since 2017. She has been an outpatient clinical documentation specialist since July 2021 for the UC Davis Health Information Management Division. She worked within the inpatient clinical documentation integrity program at Novant Health in Winston-Salem, North Carolina for six years as a inpatient clinical documentation specialist, auditor, preceptor, and physician educator for MCC/CC education. Then in 2014, she transitioned to Novant Medical Group to help build one of the first ambulatory OP CDI programs in the country.

Betty Hovey, BSHAM, CCS-P, CDIP, CPC, COC, CPMA, CPCD, CPB, CPC-I, is a nationally recognized healthcare consultant, author, and speaker that has over 35 years of healthcare experience. She has co-authored manuals on ICD-10-CM, ICD-10-PCS, E/M, and CPT coding for multiple specialty areas. Hovey is an engaging speaker that enjoys sharing her knowledge. She is a former AAPC National Advisory Board member/officer and former AAPC director of ICD-10 development and training. Betty is currently a subject matter expert and item writer for AHIMA as well as a council member on the AHIMA National Council for Excellence in Education. She holds a Bachelor's degree in health care administration and management from Colorado State.

Betty Stump, MHA, RHIT, CPC, CCS-P, CDIP, CCDS-O, CRC, serves as an Edifecs Solutions consultant focused on supporting customers seeking to incorporate technology solutions to solve the complexity of risk adjustment capture and reporting. She has an extensive professional background in healthcare consulting, working almost exclusively in the provider space focused on clinical documentation standards and regulatory expectations. She holds numerous credentials in health information, coding, and clinical documentation integrity and has a master's degree in healthcare administration.

Brittany McKinnis, RT (R), CPC, CRC, is currently serving as a risk adjustment manager at Summit Medical Group with high focus on quality patient care. Having achieved her Associate of Applied Sciences in Health Science–Radiologic Technology, she realized after five years in the profession that it was not her calling. Being involved in healthcare was still important to her, so she decided to take a chance in coding and auditing, and attained her Certified Professional Coder (CPC) certification as well as the Certified Risk Adjustment Coder (CRC) certification. She has experience in risk adjustment, ICD-10-CM coding and physician documentation, and working alongside providers to ensure every patient's disease burden is appropriately reported. McKinnis is an educator on accurate and complete documentation while adhering to CMS and ICD-10-CM coding guidelines.

Carrie Horn, MSHA, BBA, RN, CCDS, CCDS-O, CRCR, CPC, CHFP, is the system director for outpatient CDI at Baylor Scott & White Health (BSWH) in Temple, Texas. She has 27 years of nursing experience, 14 of those spent in the CDI profession leading high quality, cost-effective adult and pediatric programs while standardizing process improvement across inpatient and ambulatory CDI operations. Prior to joining BSWH as the outpatient CDI director in 2022, she spent 12 years building and transforming inpatient CDI, outpatient CDI, and provider CDI education at a large academic health system in Michigan. She has expertise in developing, monitoring, and auditing compliant inpatient and outpatient CDI infrastructure, risk adjustment, provider coding education, clinical denials, and post-payment audits. She continues to serve the ACDIS community as a member of the ACDIS Leadership Council and has presented at MACDIS, MHIMA, AHIMA, RISE, the ACDIS Outpatient Symposium, EPIC UGM, 3M, and annually in the ACDIS exhibit hall.

Danielle Wirth, BSN, RN, CCDS, CCDS-O, has been a CDI specialist since 2015. Her clinical nursing background is in pediatric critical care. Her CDI experience includes inpatient CDI, outpatient CDI, and DRG denial roles. She has been a team educator, held roles in leadership, and is experienced in developing outpatient CDI programs, including educating providers and staff.

Deanne Wilk, BSN, RN, CCDS, CCDS-O, CDIP, CCS, is a CDI education specialist for ACDIS at HCPro. She serves as a full-time instructor for the CDI Boot Camps and as a subject matter expert for ACDIS. Wilk is an accomplished healthcare professional with a diverse background in health information, medical coding, nursing, and CDI. With a strong passion for making a difference, Wilk has dedicated her career to improving the quality and integrity of clinical documentation within the healthcare environment for improved patient care. Wilk has served on the ACDIS Advisory Board and founded the Central Pennsylvania ACDIS local chapter, demonstrating her commitment to advancing the profession and sharing her expertise with others.

Denise Deter, RN, BSN, CCDS, has more than 29 years of experience in the nursing profession with the last 10 years in CDI. Prior to CDI, her background as a nurse includes medical-surgical, ICU, and emergency room. She spent multiple years contracting as a second-level review auditor and training new CDI specialists. For the last four years, she been at Valleywise Health in Phoenix, Arizona. She started there as the inpatient CDI supervisor with the last two years as the CDI manager for both outpatient and inpatient CDI. The last two years Denise’s primary focus was to build the outpatient CDI program at Valleywise Health. She currently serves as one of the Arizona ACDIS co-chairs and serves on the ACDIS Chapter Advisory Committee.

Hailey Ryfinski, RN, CCDS, is the director of outpatient CDI for Mayo Clinic Health System. She has 17 years of experience in the field of nursing, 12 years in the field of CDI, and 10 years in leadership. She has successfully transformed an inpatient CDI program for a multi-hospital health system, redesigning processes for better efficiency and effectively measuring and communicating program results to the C-suite. In 2022, she joined Mayo Clinic’s outpatient CDI operations where she leads efforts in standardized process improvement, team engagement, and data-driven interventions to deliver cost-effective and high-quality program results.

Heather Arnder, RN, BSN, MHA, MBA, CPC, CRC, CCDS-O, is an outpatient documentation specialist. She completed her undergraduate studies at Winston Salem State University in North Carolina. She received her Bachelor of Science in Nursing. She completed her master's studies at Pfeiffer University in North Carolina and received her Master of Healthcare Administration and an additional Master of Business Administration. With over 20 years of experience in cardiology, surgical, and data analytics. She also has over seven years of outpatient clinical documentation experience. Heather began at UC Davis as a contract employee starting an OP CDI program and is now a current UC Davis employee. Arnder is a member of the North Carolina AAPC chapter and ACDIS. Her career in CDI has allowed her to use knowledge from previous experience and apply that to help providers achieve outstanding care for their patients.

Jason Jobes, BA, MSPA, is the senior vice president of solutions for Norwood, where he oversees all consulting partnerships and delivery for Norwood’s healthcare partners. These partnerships focus on creating economic sustainability for the healthcare ecosystem through improved performance across the revenue cycle and value-based care terrains. Jobes holds a Bachelor of Arts from University of Hawai’i Hilo, where he graduated with a degree in economics with a minor in business administration. He also obtained his Master of Science in Predictive Analytics from Northwestern University in Evanston, Illinois.

John Stoebe, JD, MHA, has worked for Northwestern Medicine since 2018, beginning in organ transplant with oversight for the finance, procurement, and quality ratings within the various organ service lines. After review of scoring and outcomes that did not accurately represent the acuity of patients cared for or the quality of care given, Stoebe embarked on an educational investigation to enhance and represent the work being done. Eventually, he came to an understanding of the importance of clinical documentation and coding as it relates to key metrics, such as observed and expected graft failure and mortalities. After working to enhance the outcomes in organ transplant, he eventually moved on to Northwestern’s ambulatory CDI program, overseeing the scaling, implementation, and support of over 100,000 Medicare Shared Savings Program and Medicare Advantage lives and continued growth.

Kathryn DeVault, MSL, RHIA, CCS, CCS-P, FAHIMA, is the senior manager for HIM consulting services for United Audit Systems, Inc. (UASI), based in Cincinnati, Ohio. She has over 30 years of experience within the health information and coding management arena. Prior to joining UASI, she served as the senior director of HIM coding and reimbursement for AHIMA. Named one of the top ten CDI professionals by ACDIS in 2017, DeVault has been one of the primary ICD-10-CM/PCS content developers and instructors for the AHIMA ICD-10-CM/PCS Academy since 2009. She has authored and provided support for AHIMA online coding education and also served as a technical advisor for the association on ICD-9-CM, ICD-10, and CPT coding publications. DeVault has presented numerous seminars and educational sessions on coding, CDI, and HIM-related topics.

Khush Singh, MD, MHA, CPC, CRC, CPCO, CPMA, CDIP, is a trained clinician and experienced healthcare executive with more than 18 years of combined HIM and Medicare experience. Singh has served in several senior leadership roles within risk adjustment teams at Medicare Advantage health plans and overseeing Medicare, Medicaid, and commercial risk adjustment efforts. Singh leads focused engagements providing expertise in risk adjustment operations, healthcare data analytics, HCC coding, and operational compliance components of Medicare’s Part C and Part D. He has led a variety of projects, including: risk adjustment (RA) operations and process improvement; prep for Risk Adjustment Data Validation audits; retrospective chart retrieval campaigns; provider and coder education; structuring and optimizing RA programs within management services organizations and independent physician associations; due diligence audits supporting mergers and acquisitions by private equity firms; and analyses of encounter data submissions.

Lisa Norby, RHIT, CCS-P, CCDS-O, is the supervisor for outpatient clinical documentation integrity at Mayo Clinic Health System. She has over 15 years of coding and documentation experience within Mayo Clinic and has held a variety of positions including clinical documentation specialist, CDI lead, as well as professional coding and billing supervisor. Norby was instrumental in developing and expanding Mayo's OCDI program and is a strong advocate for OCDI efforts elsewhere. She has presented at ACDIS, MNHIMA, and WHIMA conferences and sits on the Wisconsin Chippewa Valley Technical College HIT Program Advisory Board. She is currently pursuing higher education at the University of Wisconsin-Stout.

Nicole Barber, RN, BSN, CCDS-O, CPC-A, is the outpatient CDI educator at Baylor, Scott, and White. She has been in CDI since 2018, when she started as a production staff member of the inpatient CDI team. Barber then transitioned to a provider education role and is now involved in outpatient CDI. She has extensive knowledge of CPT codes, ICD-10 coding, and risk adjustment.

Rachel Coon, CRC, CCDS, is an experienced educator in CMS-risk adjustment and ICD-10-CM coding/documentation guidelines. Coon is skilled in physician relations, medical compliance, ICD-10-CM, risk adjustment, coding documentation, and healthcare management and has over 20 years of experience in the medical field. As an AHIMA- and AAPC-approved ICD-10-CM trainer, Coon wields an intense passion for helping others learn about medical coding, documentation, and compliance. Coon has also spent three years in Dubai and Abu Dhabi working with the UAE government to create and implement an “audit coding methodology” for medical facilities and providers. As co-founder of the Abu Dhabi AAPC chapter and chapter president, Coon continues to promote coding compliance and training. Since returning to the U.S., Coon has served as the national educator, CDI operations manager, and now risk adjustment director in one of the largest privately owned primary care physician organizations in Tennessee.

Sandra Love, MSN, RN, CCDS, CCDS-O, CPC, is the director of solutions for Norwood in Austin, TX. She has more than 17 years of nursing experience and 11 years in CDI. Previously, Love served as a manager of outpatient clinical documentation excellence at Atrium Health Wake Forest Baptist in Winston-Salem, North Carolina, where she helped implement two pediatric CDI programs and an outpatient CDI program. A past president of North Carolina ACDIS, Love has spoken at ACDIS chapter meetings and past national ACDIS conferences. She is also a former member of the ACDIS Events Committee and ACDIS Leadership Council.

Shayna Carlson, MSN, RN, CRC, is Northwestern Medicine’s (NM) ambulatory clinical nurse educator. Prior to her current role, she worked in emergency services as an educator, overseeing staff training, compliance with regulatory bodies, quality, and proficiency of staff competency. She has developed educational training interventions utilizing multiple modalities. Using skills she learned in the emergency department, she has successfully collaborated with different specialties and arms of support at NM, including primary care, oncology and cardiology. Tara Renze is an emotional intelligence practitioner, accomplished author, and keynote speaker, who is fueled by an unwavering mission to empower individuals to embrace their authenticity and "be who they came to be." Her inspiring journey was sparked over 16 years ago by a letter her 87-year-old grandmother wrote to her infant son, containing a profound message that simply said, "Be who you came to be." In those six words, Tara found the courage to embrace her own authenticity and create, own, and live the life she desired. Now, she dedicates her life to helping others elevate their emotional intelligence to unleash their purpose, ignite their authenticity, and cultivate enduring happiness, success, love, and fulfillment across all aspects of their lives.

Teresa Caniglia, BSHS, RHIT, CCDS-O, CPC, CRC, CDEO, CRC-I, is a director of clinical documentation integrity at Molina Healthcare. Caniglia currently works with all health plans across the enterprise, educating on risk adjustment methodology, CDI scope, and documentation accuracy. She is passionate about provider and patient education surrounding annual visits, health literacy, the identification of social determinants of health, and closing the disparities in health equity. Caniglia holds an associate's degree in health information management, and a bachelor’s degree in health science from Northern Kentucky University. She is currently pursuing a master’s degree in healthcare administration at Kentucky University.

Valerie Bica BSN, RN, is the lead CDI specialist for clinical documentation integrity at Nemours Children’s Health System in Wilmington, Delaware. She has been at Nemours for 20 years. Bica has 45 years of pediatric/NICU nursing experience, including case management, care management, managed Medicaid, acute care pediatrics, pediatrics, and neonatal ICU, as well as supervised homecare for children with technology needs. She also participated in the start of the CDI program for the Nemours Children’s Health System Delaware. Bica is a co-leader of the ACDIS pediatric networking group (APDIS) and previously served on the 2015–2016 ACDIS Pediatric Respiratory Failure Work Group. In 2018, she assisted in the writing of an ACDIS white paper on pediatric heart failure criteria and has, throughout her career, presented at multiple national conferences on the topic of pediatric CDI nursing.

Viji Anchan MSN, RN, CPN, is a CDI specialist who focuses on risk adjustment for Nemours' primary care sites. With over 23 years of nursing experience, Anchan has worked in a wide variety of settings including adult medical-surgical, geriatric, and pediatric care. In her 14 years at Nemours, Anchan has gained valuable experience in various departments such as the orthopedic and transplant unit, surgical-intermediate unit, and post-anesthesia care unit. In 2019, she earned her master's of science in nursing from Wilmington University. Additionally, Anchan holds certifications as a certified AAPC risk coder and a PNCB certified pediatric nurse. For the past four years, she has been an integral part of the CDI department at Nemours, where she provides coverage for both inpatient and outpatient sites.
