About the authors:

Diane Link, RN, MHA, has over 25 years of home health and hospice experience in all levels of the industry. She has been a registered nurse for 27 years and has served as a clinician, performance improvement nurse, clinical manager, and executive director. She also has worked as a CHAP (Community Health Accreditation Program) surveyor for home health, hospice, and private duty services. She currently is a home health/hospice senior clinical consultant for BlackTree Healthcare Consulting. Her knowledge of home health and hospice regulations has enabled her to help agencies improve clinical, organizational, and performance improvement workflows and assist them in meeting regulatory requirements. Link is familiar with all aspects of managing home health and hospice organizations, including clinical management, episodic management, performance improvement, and outcome management.

Faith Williams, BS, RN, is a registered nurse with an extensive background in home health education, nursing, and healthcare administration. After spending many years in home health, she has since worked for companies who provide online education to healthcare providers and workers in the acute and subacute markets.
